  4. Resident

  5. Complex construction date approximate

  6. Balcony or raised surface: above the grade

  7. Elevation above grade approximate

  8. Construction: wood decking or other

  9. Open or opaque surface material

  10. Underfloor area: ventilated or open

  11. Decking surface

  12. Dry rot observed

  13. Connection to building observed

  14. Inspection date

  15. Inspector

  16. Attach minimum 3 photos
